All a QR code does is direct the user to a URL, so clicking any link on any website (which people can do on a smartphone) poses the same security risk.
Having said that, I'm not sure why a QR code reader is not built into cameras on Smartphones. Both Apple (iOS) and Google (Android) may think that to doing so would cost them advertising revenue somehow.
By the way, if you need a good QR code reader for you iPhone, I recommend "QR Code Simple".

A. There are PHP scripts available that allow you to detect which kind of mobile device a person is using. Google it. You can use that knowledge to redirect the person to a page made specifically for their device. Each device has a different screen size, so try to take at least the major ones into account...you could make a page for iPhones, Blackberries, and some of the major Android phones. Set strict page dimensions that coincide with the screen size. It's okay to have it longer than the size of the screen, but make sure it's not wider...people don't like to scroll sideways!
The page should be fairly simple with not too many options...use large buttons, and make the most popular features very prominent and easy to access...
